CARC 24: Verify Medicare Advantage Enrollment Before You Rebill

A Medicare claim returned with CARC 24 needs a routing check before another submission. Sending it back to the same destination without resolving enrollment can repeat the problem and consume filing time.

X12 defines active CARC 24 as charges covered under a capitation agreement or managed-care plan. This article focuses on one common scenario: a professional claim sent to Original Medicare when the patient was enrolled in Medicare Advantage.

Confirm the coverage on the service date

First Coast’s June 2026 guidance explains that these Medicare submissions can be returned as unprocessable with CO 24. It recommends checking eligibility and asking patients about new insurance coverage. If the CMS record is corrected to show no Medicare Advantage enrollment on the service date, the contractor directs providers to resubmit. An unprocessable claim cannot be appealed through the ordinary claim-appeal route.

Pull the eligibility response and compare enrollment start and end dates with the actual service date. Keep the response date and plan identification in the account notes. The card in the chart or the patient’s coverage today may not establish who was responsible for an earlier visit.

Confirm the receiving entity

For routine services that belong with the Medicare Advantage plan, verify its claim submission instructions and payer identifier. Determine whether the plan directs claims through another responsible entity. Update the account only after confirming the destination.

CMS’s July 2026 fact sheet emphasizes that Medicare Advantage plans use their own claims and payment procedures. Check the specific plan’s eligibility, coverage, and payment rules rather than applying the Original Medicare workflow automatically.

Check exceptions before changing the route

Medicare Advantage enrollment does not mean every service must be billed to the plan. CMS identifies exceptions involving hospice, some clinical-trial costs, and certain other benefits. If the encounter involves a possible exception, use the applicable service-specific instructions and obtain clarification from the responsible payer or contractor.

Do not use a blanket routing edit that sends every claim away from Original Medicare solely because enrollment is present.

Preserve the filing trail

Record the original submission, rejection or remittance, eligibility evidence, corrected destination, and next deadline. Check whether the receiving payer recognizes any filing exception; do not assume the earlier submission automatically protects timely filing.

If enrollment information appears wrong, assign an owner to resolve that record discrepancy and a follow-up date. Avoid repeatedly sending unchanged claims while the underlying record remains unresolved.

After a correctly routed submission, track acceptance and adjudication. Separate an unprocessable Original Medicare claim from a later plan payment denial, since the available next steps may differ.

Prevent the next routing error

Review a small sample of CARC 24 accounts each month. Separate outdated coverage records, incorrect payer identifiers, enrollment disputes, and service-specific exceptions. Use confirmed findings to improve registration-to-billing handoffs and eligibility checks before submission.

How Syntra can help

Syntra’s chart-and-claim audits can help your practice distinguish coding and documentation issues from claims needing an eligibility or routing review. Use validated findings to establish pre-bill review guidelines and clear handoffs. Registration and billing staff should still confirm enrollment, payer identifiers, and exception-specific submission instructions before releasing the claim.
